Quick summary
Ebglyss is a prescription biologic containing lebrikizumab. In current EU, UK and US product information, it is used for moderate-to-severe atopic dermatitis in adults and adolescents aged 12 years or older who weigh at least 40 kg, under jurisdiction-specific treatment criteria. The 250 mg pre-filled pen is injected under the skin.
Important: The loading and maintenance schedule is not simply one injection at week 2 followed by monthly dosing. The official schedule includes two 250 mg injections at both week 0 and week 2, then one injection every two weeks until an adequate response is achieved. Follow the prescription and Instructions for Use supplied in your country.
What is Ebglyss?
Ebglyss is the brand name for lebrikizumab, a monoclonal antibody that binds to interleukin-13 (IL-13). IL-13 contributes to the inflammation involved in atopic dermatitis. Blocking its signalling can reduce eczema severity and itching.
Each single-use pre-filled pen contains 250 mg in 2 mL, equivalent to 125 mg/mL. A pre-filled syringe presentation also exists; device instructions are not interchangeable.
What is Ebglyss used for?
In the European Union and United Kingdom, Ebglyss is authorised for moderate-to-severe atopic dermatitis in adults and adolescents aged 12 years and older who weigh at least 40 kg and are candidates for systemic therapy.
In the United States, Ebglyss is authorised for the same age and weight group when atopic dermatitis is not adequately controlled with topical prescription therapies or when those therapies are not advisable. These criteria should not be presented as a universal indication.
Ebglyss can be used with or without topical corticosteroids. Topical calcineurin inhibitors may be used for particular areas when advised by the prescriber.
Official dosage schedule
The current EU, UK and US labels use the following schedule for eligible adults and adolescents:
- Week 0: 500 mg, given as two 250 mg injections.
- Week 2: 500 mg, again given as two 250 mg injections.
- After week 2: 250 mg every two weeks through week 16.
- After an adequate clinical response: 250 mg every four weeks as maintenance.
EU and UK product information advises considering discontinuation if there is no clinical response after 16 weeks, although some people with a partial initial response may improve with every-two-week treatment through week 24. Treatment response and any change to the schedule must be assessed by the prescriber.
How to use the pre-filled pen
A patient or caregiver may give Ebglyss after a healthcare professional decides that self-injection is appropriate and provides training. Read the country-specific Instructions for Use each time the device or packaging changes.
- Inject under the skin of the thigh or abdomen, avoiding the area around the navel described in the leaflet.
- A caregiver may also inject into the back of the upper arm.
- Rotate sites and avoid skin that is tender, damaged, bruised or scarred, or affected by atopic dermatitis.
- When two injections are required, use two different injection sites.
- Do not shake the pen, and do not use it if the medicine is frozen, cloudy, discoloured or contains particles.
- Place each used pen in an approved sharps container.
Do not rely on a general web article for device operation. The illustrated Instructions for Use supplied with the exact pen are the controlling instructions.
Storage
Store Ebglyss in a refrigerator at 2°C to 8°C, in its original carton to protect it from light. Do not freeze it and do not expose it to heat or direct sunlight.
Current EU, UK and US information permits storage at room temperature up to 30°C for no more than 7 days. Once removed from refrigeration and stored under these conditions, it should not be returned to the refrigerator and must be discarded if not used within the permitted period. Check the leaflet supplied with your medicine because local packaging instructions remain authoritative.
Side effects and warnings
Common adverse reactions in official product information include eye inflammation—particularly conjunctivitis—reactions at the injection site and dry eye. The US label also lists herpes zoster among common adverse reactions. Keratitis and allergic conjunctivitis can occur.
Seek urgent medical help for signs of a serious allergic reaction, such as breathing difficulty, swelling of the face, mouth or tongue, fainting, dizziness, widespread hives or a severe rash. Stop using the medicine until urgent advice is obtained.
Report new or persistent eye symptoms. Official information recommends assessment when conjunctivitis does not resolve with standard treatment or when symptoms suggest keratitis.
Vaccines and parasitic infections
Live and live-attenuated vaccines should not be given concurrently with Ebglyss. Age-appropriate vaccinations should be reviewed before treatment. Official labels also advise treating a pre-existing helminth infection before starting Ebglyss and reassessing treatment if an infection develops and does not respond to antihelminth therapy.
Pregnancy and breastfeeding
Human pregnancy data are limited. EU and UK information says it is preferable to avoid lebrikizumab during pregnancy as a precaution, while the US label describes insufficient human data to evaluate risk. Breastfeeding decisions require an individual assessment of benefit and potential risk. Discuss pregnancy plans and breastfeeding with the prescriber rather than applying one jurisdiction’s wording universally.
Ebglyss and Dupixent are different medicines
Dupixent (dupilumab) and Ebglyss are biologic medicines that may be used for atopic dermatitis, but they contain different antibodies, have different authorised indications across jurisdictions and use different schedules. They are not interchangeable. A clinician selects treatment using disease severity, age, weight, previous therapy, comorbid conditions and local authorisation or access criteria.
Missed dose or possible overdose
For a missed dose, current labels advise giving it as soon as possible and then resuming the scheduled dosing pattern. Confirm the next date with the prescriber or pharmacist if there is any uncertainty.
There is no specific antidote for lebrikizumab overdose in the official information reviewed. A person who receives more than prescribed should contact the treating service for monitoring and advice; do not take extra injections to correct a schedule without instructions.
Frequently asked questions
Is Ebglyss used for mild eczema?
The official indications reviewed are for moderate-to-severe atopic dermatitis under specific systemic-treatment or topical-treatment criteria. A clinician determines whether those criteria are met.
How many pens are needed for the first doses?
The 500 mg doses at week 0 and week 2 each require two 250 mg pens and two separate injection sites. Later scheduled doses use one 250 mg pen.
Can Ebglyss be used in someone under 12?
The current EU, UK and US indications reviewed begin at age 12 and require a body weight of at least 40 kg. Safety and effectiveness below those thresholds have not been established in those labels.
Can the pen be put back in the refrigerator?
Not after it has been removed and stored at room temperature under the 7-day allowance described in the official information reviewed. Record the removal date and follow the leaflet supplied with the pen.
